Understanding the grading of fatty liver disease is critical because it tells you how much fat has built up and how much damage has occurred. While early stages are often "silent" with no symptoms, they are the most important times to take action. Here is a breakdown of the four main stages, from mild fat buildup to severe scarring.
Stage 1: Simple Fatty Liver (Steatosis)
This is also known as Grade 1 Fatty Liver. At this stage, fat begins to build up in the liver cells, but there is no inflammation or damage yet. Most people have no symptoms. The good news? This stage is fully reversible with a healthy diet and regular exercise.
Stage 2: NASH (Non-Alcoholic Steatohepatitis)
In Stage 2, the excess fat causes the liver to become inflamed and swollen. This is more serious because the irritation can start to damage liver tissue. You might begin to feel tired, weak, or experience a dull pain in the upper right side of your abdomen. While still reversible, it requires immediate attention to stop further progression.
Stage 3: Fibrosis (Scarring)
Long-term inflammation leads to Fibrosis, where persistent damage causes scar tissue to form around the liver and nearby blood vessels. The liver can still function, but its capacity is reduced. While mild fibrosis can sometimes be improved, deep scarring is very difficult to reverse and needs intensive medical care.
Stage 4: Cirrhosis
This is the most severe and irreversible stage. The liver is heavily scarred, lumpy, and shrunken. At this point, the liver cannot work properly, leading to symptoms like jaundice (yellow skin), swelling, and severe weakness. This stage is life-threatening and may eventually require a liver transplant.
